To file a complaint, please fill out the intake form as completely as possible. We will call you if we need information in addition to what is on this form. We will respond as quickly as possible. Please be aware that our limited resources mean that we can accept very few cases.

Also, because there may be some delay in getting back to you, make sure to seek advice from an attorney about time deadlines that might apply to your case. Do not wait to hear from us before taking action with regard to your case. We cannot give you any advice about your case, and you are responsible for meeting all deadlines.

Please do not submit any original documents when you request assistance. Because we receive so many requests for assistance, we cannot return any materials that you submit to us supporting your intake questionnaire.
